Social Worker
The incumbent develops and implements screening methods for the veteran population in assigned areas in order to identify those who need social work intervention.
Formulates social treatment goals with patient/family based on the psychosocial assessment.
Selects and uses treatment methods appropriate to the unique needs of each patient.
Manages a normative workload in the established program.
Records social work activity in the medical records promptly, accurately, and concisely.
Integrates social work services into the multidisciplinary program of assignment, making certain that social work services are related to the goals and plans of the patient and team.
Establishes and maintains systematic and effective working relationships with personnel of the clinic and community agencies relevant to the area of assignment, to exchange information, arrange referral, resolve problems in service delivery or develop new resources.
Assumes responsibility for professional development and assessing and monitoring his/her level of practice.
